This saint is customarily portrayed in a Discalced Carmelite habit (mantle, broad scapular, For monks and friars, a long, broad bib as seen here. For lay people, a simple pair of illustrated cloth tags worn front-and-back against the skin. sandals). Other portraits usually add a cross, referring to the saint's name. The left hand seems to have once held a pen; at this time only a small broken part of the shaft remains. The pen and book would refer to his writings on the contemplative life.
